The College of Liberal Arts and Sciences Early Start Application Form

The College of Liberal Arts and Sciences offers 11 different major-specific Early Start programs to give you the tools you need to be successful at ASU. Each program is designed to create an immersive, integrated experience of college life where you'll learn what to expect for the upcoming fall semester.

Program Terms and Agreement

This form serves as documentation that The College of Liberal Arts and Sciences Early Start participants are aware of, and agree to abide by, The College Early Start policies and procedures listed below. Failure to do so may result in immediate removal from the Early Start program or other disciplinary actions deemed appropriate by University Housing, The College of Liberal Arts and Sciences or The Office of Student Rights and Responsibilities.

  1. Students will abide by the ABOR Student Code of Conduct, University Housing, and The College of Liberal Arts & Sciences policies and procedures.
  2. Violation of the following "Zero Tolerance" policies will result in immediate dismissal from the program: * Students will not participate in any activities involving the use or presence of alcohol or other drug substances. * Students will not possess any items considered to be weapons of any sort. * Students will respect person and property. Damage, theft and physical violence are not tolerated.
  3. ASU is tobacco-free. Students may not smoke cigarettes or other tobacco products in the residence halls, classrooms or during The College Early Start activities.
  4. Students will attend and be on time for all events on The College Early Start Schedule, including, but not limited to, classes, presentations, meetings, study sessions, residence hall meetings (if participant is living on campus) and activities.
  5. University Housing, Student Rights and Responsibilities and The College of Liberal Arts and Sciences reserve the right to remove students from the residence halls and/or campus.
  6. If you are no longer able to participate in the program, you will need to cancel no later than Monday, July 15th, 2024 by contacting thecollegeearlystart@asu.edu.
Photo and Video Release

I give permission to release my name, photograph, video and related information about me or a photographed/videotaped event in which I am pictured to the news media. I further give permission for ASU Public Relations and The College of Liberal Arts and Sciences to use this information and/or photograph in university publications and publicity in support of the University and The College Early Start program.

Health Emergency Care - Permission to Transport/Treat

In the event of a medical or psychological emergency, I understand that The College Early Start or University Housing staff will obtain emergency services, (i.e., ambulance, emergency room care, EMPACT crisis team, doctor care) to respond to the well being of The College Early Start participants. The ASU Health and Wellness Center will be utilized during regular business hours in non-emergency situations.
